Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. On the day of your first appointment, please arrive 15 minutes early to fill out our New Patient Forms. These will be provided to you, and will assist us in providing you with the best care possible. The forms cover some of your personal information, basic medical information and history, and any dental insurance information.
Upon arrival, check in with our friendly office staff, and we will set you up in our comfortable waiting room before you are introduced to one of our hygienists and to Dr. Greg.
What to Bring
To streamline your waiting room experience, we ask that you bring these materials with you to your first appointment.
Please bring: a list of any medications you may be on and your dental insurance card if you have one.
